First appointment Prior to seeing your physician, we ask that you complete a questionnaire noting vital health information such as medications, allergies and prior injuries. Ongoing appointments For your enhanced comfort and relaxation please arrive 15 minutes prior to your scheduled appointment. If you need to change your appointment or are going to be late please let our office know as soon as possible. We promise to try hard to adhere to the appointment schedule. Cancellations without 24-hour notice and no show appointments are charged at full value. There are times when our physicians must respond to emergencies and we appreciate your understanding. You may call ahead to make sure your appointment is on schedule. Parking is always available and convenient and it is on the same level as our clinic. Paying for your care There is no insurance coverage for most of the treatments provided by Associated SkinCare. We ask that you pay for your treatment at the time of your appointment. Cosmetic surgery is not usually covered by insurance so we ask for payment two weeks in advance of your appointment for surgical procedures. We accept cash, checks, and Visa, MasterCard, and American Express credit cards. At the time of booking, a credit card number is required to secure your reservation; payment will not be processed until check out. All prices are subject to change without notice. Gift certificates can be used toward any service, product, or surgical procedure; they are not redeemable for cash. Payment plans available Associated SkinCare is pleased to offer CareCredit® payment plans to make it easier for our patients to get the cosmetic enhancements they want, when they want them. CareCredit is available for procedure fees up to $25,000, upon request and approval. Depending on your procedure, you can conveniently pay over time with either a No Interest Payment Plan or a Low Interest Payment Plan. Choose whatever fits comfortably into your lifestyle and budget.
